Paracetamol, Pan D among 53 medicines to fail drug regulator’s quality test, safety concerns rise

More than 50 medicines including paracetamol have been found to fail. These tests were conducted by India’s drug regulator Central Drugs Standard Control Organization i.e. CDSCO. CDSCO has recently released a monthly report. In this, 53 medicines including paracetamol, Pan D and calcium supplements have been declared not to be of standard quality. This has raised concerns about the safety of their use.

In its August 2024 report, the central drug regulator has placed paracetamol, vitamin D and calcium supplements, high blood pressure medicines and some anti-diabetes tablets in the NSQ alert category of not conforming to standard quality.

The medicines that failed the quality check include Vitamin C and D3 Tablets, Shelcal, Vitamin B Complex, Vitamin C Softgel, Anti-Acid Pan-D, Paracetamol Tablet (IP 500 mg), Anti-Diabetic Drug Glimepiride and High Blood Pressure Drug Telmisartan.

These products were manufactured by several companies, including Hetero Drugs, Alkem Laboratories, Hindustan Antibiotics Limited i.e. HAL, Karnataka Antibiotics and Pharmaceuticals Limited, Pure & Cure Healthcare and Meg Lifesciences, among others.
Metronidazole, a commonly used drug for stomach infection, which is manufactured by HAL, also failed the quality test. Similarly, Shelcal, a popular calcium and vitamin D3 supplement, has also been put in the NSQ alert category.

Dosage Instructions (suitable for babies to adults) of Allium cepa
For the home treatment of acute and self-limiting complaints take one pill or five drops of the remedy every 1 minute to 4 hours (1 minute for intense or emergency symptoms (plus seek emergency help), 4 hours for milder ones). Anyone who has cut up raw onions knows what their effect upon the eyes and nose is -irritation, which causes violent sneezing and lachrymation. Then, if the homeopathic law of cure is true, it ought to be a good remedy for coryza, and so it is; but like every other remedy, it cures its own peculiar and characteristic form of the disease.


It has also profuse lachrymation, with burning, biting and smarting of the eyes, but the discharge is bland; that is, it does not make the eyes sore afterwards.


There may or may not be headache; if there is it is, like the coryza, worse in warm room or evening and better in open air.


I have found it particularly useful in children when the profuse coryza or cold extended downward to the bronchi, with a like profuse secretion in the bronchial tubes, with much coughing and rattling of mucus.


Before Cepa came into homeopathic use we used to give Euphrasia when there was profuse coryza and lachrymation.


The difference between the two remedies is, that with Cepa the nasal discharge is acrid and the lachrymal bland, while exactly the reverse is true of Euphrasia.


The action of the remedy seems to be primarily in the nose with the one and in the eyes with the other, and thus we must learn to differentiate between all remedies.

आँखें - गाढ़ा, अधिक, पीला, मन्द स्राव, अनुत्तेजक । आँखों में खुजली और जलन । अधिक जलस्राव और श्लैष्मिक स्राव, पलक सूजे, चिपके हुए। बिलनी । नेत्र छिद्र की शिराएँ बहुत बढ़ी हुई। नवजात शिशु का नेत्रप्रदाह । मन्द नेत्रप्रदाह, मन्दाग्नि के साथ, गरम कमरे में अधिक ।

नाक-जुकाम, दाहिना नथुना बन्द, नाक की जड़ पर दाब । सूँघने की शक्ति लोप । नाक के भीतर बढ़ी हुई, हरी, घृणित खुरण्ड । शाम को नाक बन्द हो जाए। पीला श्लेष्मा, सुबह को अधिक। बुरी गन्ध, जैसा पुराने जुकाम में होता है। नाक की हड्डियाँ वेदनापूर्ण ।

चेहरा-दाहिनी तरफ का स्नायु-शूल, अधिक आँसू निकलने के साथ। नीचे का होंठ फूला हुआ जो मध्य में चिपटा हो। आँखों के घेरों के ऊपर शूल जो सन्ध्या से आधी रात तक रहे, दर्द के साथ शीत ।

Mpox in india

India reported its first case of clade 1b of mpox, in a 38-year-old man in Kerala, who travelled from the United Arab Emirates. Clade 1b is the fast-spreading variant of mpox, which prompted the World Health Organisation to declare it a public health emergency of international concern (PHEIC).

The earlier case of mpox reported in India was from Delhi. A 26-year-old resident of Haryana's Hisar had tested positive for clade 2 strain of the mpox.


Mpox is a viral infection caused by the monkeypox virus, a species of the genus Orthopoxvirus. There are two variants of mpox that are distinct in nature that influence the severity of the disease and its impact on affected populations.

These strains are: clade 1b (The Central African (Congo Basin) strain) and clade 2 ( West African strain).

This strain causes symptoms like skin rash that is followed by pus-filled blisters, high fever, headaches, muscle aches, back pain, low energy, swollen lymph nodes and sore throat.

The blisters can appear on the palms of the hand and soles of the feet, face, mouth, throat, groin and genital areas, and anus.


The mpox strain is transmitted via skin-to-skin contact. It can also include being face-to-face with someone who has mpox (such as talking or breathing close to one another, which can generate infectious respiratory particles).

According to the WHO, people with multiple sexual partners are at higher risk of acquiring mpox. Besides this, people can also contract mpox from contaminated objects such as clothing or linen, through needle injuries in health care, or in community settings such as tattoo parlours.

People with mpox can pass the disease on to others until all sores have healed and a new layer of skin has formed.

Children, pregnant women and those with weak immune systems are at a high risk of contracting mpox.

The clade 1b strain is known for its higher virulence, leading to more severe symptoms and complications. Patients infected with the Central African strain are at greater risk of experiencing serious health issues, including a higher mortality rate of up to 10%.

उपरोक्त चिमटियों में खिसकने वाला ताला (Sliding lock), दो फलक (Blade), पिंडली (Shank), हत्या (Handle) आदि संरचनाएँ होती हैं। फलक व पिंडली वाले भाग में दो स्थानों पर वक्रीय मोड़ पाये जाते हैं-

चिमटी प्रसव की पूर्वाकांक्षाएँ (Prerequisites for Forceps delivery) - गर्भाशय ग्रीवा (Cervix) पूर्णतः फैला हुआ (Completely dilated) होना चाहिए।

• गर्भ के सिर की सही-सही स्थिति (Exact position) का ज्ञान होना चाहिए।

• गर्भ झिल्लियाँ फटी हुई ( Ruptured) होनी चाहिए।

• गर्भ शीर्ष प्रस्तुति (Vertex presentation) या चेहरा प्रस्तुति (Face presentation) में होना चाहिए।

• ठोड़ी (Chin) आगे की ओर, सिर फँसा हुआ, विशेषकर मूलाधार (Perineum) पर होना चाहिए।

• किसी भी परिस्थिति में कपाल श्रोणीय विषमानुपात (CPD) नहीं होना चाहिए।

चिमटी प्रसव की स्थितियाँ (Conditions for Forceps delivery)- मूलाधारी कठोरता (Rigid perineum), पश्चकपाल-पश्चवर्ती अवस्था (Occipito-posterior position) का लगातार बना रहना, गहरी अनुप्रस्थ रोक (Deep transverse arrest), असामान्य प्रस्तुति, विशेषकर नितम्ब प्रस्तुति (Breech presentation), गर्भाशयिक जड़त्व (Uterine inertia) आदि के कारण प्रसव के द्वितीय चरण में विलम्ब होने पर।

गर्भावस्था की किसी जटिलता से प्रसवा पीड़ित हो; जैसे-

साधारण विचार-रोग-बीजाणु यक्ष्मा के कारण बाजरे के दाने की तरह फुंसियाँ टायफायड फेफड़े- यक्ष्मा से ग्रंथि-प्रदाह-आन्त्र क्षय-आँतों में क्षयरोग फेफड़ों में क्षयरोग फेफड़े का पुराना क्षयरोग ।

साधारण विचार - बचपन में साधारण क्षयरोग उसके फेफड़ों में क्षयरोग के बीजाणुओं के जमा होने से नहीं होता। बच्चे के शरीर के हर अंग और तंतु में यक्ष्मारोग की प्रवणता रहती है, ऊपरी तौर पर इसका निदान निकालना बहुत ही कठिन है। जब क्षयरोग बच्चे के शरीर में प्रविष्ट हो जाता है तो उसके फेफड़े, मस्तिष्क, स्थानीय ग्रंथियों, चमड़ा तथा हृदय आक्रान्त हो जाते हैं। ऐसे स्थलों में निदान जानना सहज है, किन्तु मध्यान्त्र ग्रन्थियों, फेफड़ों के भीतरी झिल्लियों, अन्त्रावरक झिल्लियों, आँतों, यकृत, प्लीहा और गुद्दों में क्षयरोग हो जाने पर निदान निकालना बहुत ही कठिन हो जाता है।

शिशु के बचपन में साधारण क्षयरोग प्रायः नहीं होता अर्थात् बच्चे के जीवन के प्रथम कुछ महीनों में; किन्तु दूसरी ओर दूसरे या तीसरे साल ऐसे क्षयरोग का आक्रमण हो सकता है। पेरिस के अस्पतालों के बच्चों के विभाग में २१ प्रतिशत बच्चे इसी क्षयरोग से मर जाते हैं और अन्य बड़े-बड़े नगरों में बच्चे के प्रथम पाँच वर्षों के भीतर मृत्यु-संख्या काफी होती है।

इस विषय में माता-पिता से क्षयरोग का संक्रमण नहीं माना जाता। प्राचीन समय में जैसा समझा जाता था, वैसा आजकल माता-पिता से बच्चों में संक्रमण स्वीकृत नहीं है। बच्चे के शरीर की प्रकृति ही वैसे रोगाक्रमण का कारण माना जाता है। किसी भी कारण से उसके शरीर में वैसे कठिन रोग का संक्रमण हो सकता है। इसका भेद जानना बहुत ही आवश्यक कार्य है। जहाँ पेट में बच्चा रहते समय माता का यक्ष्मा- रोग अत्यन्त बढ़ गया हो, वहाँ वह पैतृक समझा जा सकता है। यह एक देखने की बात है कि कितनी स्त्रियाँ स्वस्थ बच्चों को जानती हैं? जो जीवन भर स्वस्थ रहते हैं।

कुछ ग्रन्थकार विरोध प्रकट करते हैं और कहते हैं कि वैसा विष बच्चों के शरीर में माता के द्वारा प्रविष्ट हो जाता है। किन्तु वह जीवन के सन्धि- स्थल में आने तक निष्क्रिय रहता है, जैसे कि यौवन काल या स्त्रियों के लिए बच्चा जनने का समय, जब कि वह रोग बढ़ जाता है। किन्तु सौभाग्य से मनुष्य के परिवार में यक्ष्मा रोगाक्रान्त, माता-पिता से उत्पन्न होकर भी बच्चे अच्छा स्वास्थ्य और दीर्घजीवन प्राप्त करते हैं, यदि वे स्वास्थ्यकर वातावरण में पालित होते हैं। अन्तिम जीवन में भी वे वैसे रोगः की वृद्धि की प्रवणता से मुक्त रहते हैं।

रोग का संक्रमण, टीका, चोट, रगड़ अथवा घाव के जरिये यक्ष्मा- रोग के बीजाणु दूसरे के शरीर में प्रविष्ट हो जाते हैं, टीका के द्वारा तो सीधे रोग पहुँच जाता है। यक्ष्मा-रोगी जहाँ थूकते हैं, वह थूक सूख जाने पर वहाँ के रोग-बीजाणु हवा में फैल जाते हैं। इस विचार को अधिक बढ़ाने की आवश्यकता नहीं है, क्योंकि प्रायः सर्वत्र छोटे बच्चों से लेकर बूढ़ों तक में साँस के द्वारा रोग-संक्रमण के अनेक प्रमाण मिले हैं। खास- कर जिन बच्चों या उमरदार आदमियों में यक्ष्मा-रोग बढ़ चुका है, उनके सामने रहने से छोटे-छोटे बच्चों में यह रोग संक्रमित हो जाता है।

ऐसा रोग-संक्रमण खाद्य के द्वारा भी होता है और आजकल यह सिद्धांत स्वीकृत हो चुका है कि दूध के द्वारा छोटे बच्चों में यक्ष्मा-रोग के बीजाणु पहुँच जाते हैं। परीक्षा से जाना गया है कि दूध देने वाले पशुओं *में यक्ष्मा के बीजाणु व्यापक रूप से रहते हैं। यहाँ तक कि मनुष्य या पशु किसी के भी फेफड़ों में जब यह रोग स्थायी हो जाता है तो उनके दूध से वे बीजाणु बच्चों के शरीर में प्रविष्ट हो जाते हैं। अणुवीक्षण यन्त्र या जाँच की नली से निश्चित रूप से जाना गया है कि दूध के द्वारा रोग- संक्रमण एक साधारण बात है। 'पूर्वी प्रदेशों के दुग्धशालाओं के पशुओं में १० या १५ प्रतिशत पशु इस रोग से ग्रसित रहते हैं, यह बात डा० ओस्लर ने प्रमाणित की है। इस कारण बहुत ही सावधानी से दूध देने वाली गायों की परीक्षा पहले से ही कर लेनी चाहिए। बहुत-सी गायों से दूध लेना उचित नहीं है, क्योंकि उनमें २-१ रोगाक्रान्त भी हो सकती हैं। यदि दुग्धशाला की एक भी गाय यक्ष्मा-रोग से ग्रसित हो तो वहाँ का सारा दूध दूषित हो जाता है। बहुत-सी दुग्धशालाओं में सुखण्डी रोगाक्रान्त ज्वर से ग्रसित या यक्ष्मा रोग बीजाणु से भरी हुई १-२ गाय अवश्य रहती हैं।

MEASLES

An acute infectious viral disease characterised by 'Koplik's spots', high fever, nasal catarrh, cough and blotchy rash.

CLINICAL FEATURES

Prodromal stage: Duration: 1-4 days.

Symptoms

Onset:

Sudden.

High fever.

Coryza.

Sneezing.

Lachrymation.

Photophobia.

Hoarseness.

Hacking cough.

ETIOLOGY

Distribution: Worldwide.

Season: Common during winter.

Causative organism: Measles virus.

Source: Human respiratory discharges.

Predisposing causes: Poor sanitation, poor nutrition and overcrowding.

Transmission: By direct contact, droplet infection.

Incubation period: 8-14 days.

Age: 3-5 years.

Signs

Temperature: 40 °C-41 °C.

Pulse: relative bradycardia.

Face: congested.

Eyes: congested.

On 2nd day:

Koplik's spots become apparent (Koplik's spots are tiny whitish spots At buccal mu- cous membrane opposite upper second molar).

On 3rd day:

Temperature: normal.

Exanthematous stage

Duration: 4-7 days.

Symptoms

Fever rises again on 4th day.

Headache.

Retro-orbital pain.

Lachrymation.

Photophobia.

Hacking cough.

Rash appear on 4th day.

Signs:

Temperature: 40 °C-41 °C.

Pulse: fast.

Face: puffy.

Eyes: congested.

Rash: Pink, maculo-papular. Appears first on forehead and spreads to whole body within few hours. Blanches on pressure.

Convalescent stage

Duration: 7 days.

Symptoms:

Fever subsides within 1-2 days.

Rash fades from above downwards.

Brown stained desquamation.

Desquamation subsides within 1-2 weeks.

COMPLICATIONS

Immediate

Otitis media.

Bronchitis.

Pneumonia.

Late

Bronchiectasis.

Subacute sclerosing panencephalitis (after 5- 7 years).

(A rare, chronic progressive and fatal neuro- logical complication of measles. It is characterised by insidious onset with intel- lectual deterioration, apathy and clumsiness, followed by myoclonic jerks, rigidity and de- mentia in children and adolescents. EEG. is characteristic; shows bursts of triphasic slow waves.)

PROGNOSIS

Good.

GENERAL MANAGEMENT

Bed rest till fever and rash disappear.

Activities like reading, T.V. watching must be restricted till conjunctivitis subsides.

Hydrotherapy during high fever, plenty of fluids should be given to the patient.

Avoid very cold things.

PROPHYLACTIC MEASURES

Immediate

Isolation.

Children should be sent to school 7 days after defervescence.

Prevention of close contacts.

Hygienic disposal of infected secretions.

Long term

Immunisation (as per national health programme).

Improve general health.

MEDICAL TREATMENT

Prodromal state

Aconite: First stage of measles, sudden high fever. History of exposure to cold air. Child is restless, wants to be carried. Bright red rough rash. Redness of the eyes. Dry, barking, croupy cough, pulse rapid. Very thirsty for cold drinks. Chilly.

Belladonna: Cold, fever high.

Euphrasia: Early stage of measles. Eyes are sensitive to light. Constant hot irritating lachrymation. Bland nasal discharge. Measles rash. Pain in the forehead. Worse; sunlight, indoor, warm room. Better open air, blinking, wiping the eyes.

Ferrum phosphoricum: Fever with congestion of chest.

Gelsemium: Dizzy, dull, drowsy. Bodyache. Fever. Irritating, watery nasal discharge.

Malignant measles

Arsenicum album: Malignant or black or haemorrhagic measles. Sinking of strength. Diarrhoea, delirium, restlessness. Debility. Petechiae. Offensive and exhausting stools.

INFLUENZA

Influenza is an acute infectious viral disease, characterised by fever, severe body-ache, coryza, sore throat and severe prostration.

CLINICAL FEATURES

Symptoms

Duration: 7 days.

Onset:

Sudden.

Fever: Moderate to high with chills.

Headache.

Bodyache.

Bone pains.

Profound prostration.

Coryza.

Sore throat.

Cough, with or without expectoration.

Lachrymation.

Good.

GENERAL MANAGEMENT

Complete bed rest during fever. In elderly patients convalescence may be prolonged. Plenty of fluids should be given to the patient. Well balanced, nutritive and easily digestible diet should be given.

PROPHYLACTIC MEASURES

Immediate:

Isolation. Avoid crowded places during epidemics, like cinema hall, school, etc.

Hygienic disposal of infected secretions.

Long term:

Immunisation.

Improve general health.

MEDICAL TREATMENT

Aconite: Precipitated by exposure to dry cold air, or chilling of body when warm and sweaty. Chilliness. Redness of the face. Pressing headache. Anxiety and restlessness. Skin dry, violent thirst, full bounding pulse. Sweating relieves (Allium cepa) Profuse catarrhal coryza. Sneezing, running nose and irritative cough. Face is swollen and looks inflamed.

Arsenicum album; Copious flow, prostration and paroxysmal coryza. There iz burning copious watery and excoriating secretion. Thirst for a small quantity of water. Acrid coryza dropping from the tip of the nose. Headache; worse from heat, better from cold applications. Better in the warmth of the bed. Great restlessness, anxiety worse at night. Langour and excessive exhaustion.

Belladonna: General dry heat with chills. Little or no thirst. Cold extremities and hot head. Throbbing headache. Eyes red and glistening.

 Homeopathic medicine for meningitis 

Arum Metallicum- Symptoms like high fever, red face, headache and unconsciousness etc. are seen in it. parents suffering from syphilis

Arnica-

Arnica works well in the treatment of diseases caused by injuries. It is appropriate to take this medicine when the upper part of the body feels hot and the lower part is cold, excessive delirium, severe headache, unintentional discharge of stool etc. 6, 30.

Aconite: Using it at the beginning of fever prevents the fever from increasing. Its use is beneficial in case of restlessness, thirst etc. 30.

Apis-Nidra is a beneficial medicine in case of sudden startle of the child during sleep, delirium, unconsciousness, weakness, unsteadiness and the color of the face sometimes being red and sometimes pale. 6x, 30, 200.

Apocynum Canna - obstruction or absence of urination,

This is the best medicine for symptoms like continuous shaking of one hand and one leg, oblique vision, head being too big for others etc. 2x, 3x.

Amon Carb - When the child's body becomes as cold as ice, the pulse disappears and the skin of the body turns blue, then this medicine should be used. 6x.

Opium - It gives very good results when the face is hot, the lower part of the body is cold, drowsiness, bloating in the stomach, rattling in the chest, eyes half-open in pain. 3x, 30, 200.

Calcarea Carb-Child screaming without reason, frequent urination.

This is an effective medicine for symptoms like bad smell, bones not joining the head quickly, excessive sweating in the upper part of the head and late eruption of teeth etc. 30, 200.

Calcarea Fas - Passage of green, smooth and hot stool, eating-Drinking cold things increases flatulence, the child cannot keep his head straight due to sunken stomach, weakness of the spine and it is beneficial in places of extreme dryness. 6 trituration, 200.

Cuprus Mate - Stiffness in the child, fingers and toes get tied, pupils of the eyes keep rotating all around. In such a situation, this medicine is very beneficial. 6x, 30.

Glonon-It is especially useful in the cases of congestion in the brain, headache and encephalitis caused by exposure to strong sunlight. 6x, 30.

Zicum metallicum - hot head, headache, lightheadedness

It is seen in conditions like not being able to bear it, getting startled due to fear, continuously shaking the legs, etc. 30, 200.
